You confirm this declaration by ticking the Student Declaration box at checkout and typing your full name as your electronic signature. If anything in it is not true, please contact us before you book. It forms part of our Terms and Conditions.
I confirm that
- My details. I am at least 18 years old. The name, address, email address and other details I give are true and complete, and I am the person who will study the course. I will not let anyone else use my account.
- Entry requirements. I have read the entry requirements on the course page (for example anatomy and physiology, or Core of Knowledge) and I meet them. I understand that my certificate may be withheld, and a Practical Day refused without refund, if I do not.
- Insurance and the law. I understand that a certificate is not a licence to practise. Before I treat any client I will hold suitable professional indemnity and public liability insurance, check what my insurer and my local authority or regulator require, and follow the law, including medicines law. I know that this course does not allow me to obtain, prescribe or supply prescription only medicines.
- My health and safety. I will tell the trainer before any Practical Day about any health condition, medication, allergy, pregnancy, skin condition or sensitivity to light that could affect me or others. I understand that practical training involves live models and real equipment, including lasers, intense pulsed light and other energy devices, and I accept the risks that are explained to me, subject to the Terms and Conditions.
- Safe behaviour. I will follow the instructions of trainers, the laser safety Local Rules, the health and safety rules, the dress code and any requirement to wear protective equipment. I will not attend under the influence of alcohol or drugs, and I will not use any equipment unless a trainer tells me to.
- Models and confidentiality. I will treat models and other students respectfully, keep any personal information I learn about them confidential, and not photograph, film or record anyone without permission.
- Honest work. I will complete all quizzes and assessments myself, without copying, impersonation, help from another person or an automated tool unless I am told it is allowed.
- Course materials. I will keep my login private, use the online materials only for my own study, and not copy, record, share, resell or publish them or use them to train artificial intelligence.
- Responsibility after the course. I understand that I am responsible for my own consultations, consent, records, clinical governance, treatments and insurance after the course, and that no course guarantees any result, employment or insurance cover.
- Information about me. I understand that Skintastic Aesthetic Lasers Ltd will record that I signed this declaration, and will share my name and completion details with The CPD Group, and with an awarding organisation where my course is a regulated qualification, as set out in the Privacy Policy.
- Terms. I have read and understood the Terms and Conditions, including the cancellation, refund, Practical Day and liability clauses.
